Please note you cannot save data typed into the Course Application Form. To email and print: please save the form on hard disk then proceed to print or attach to an email. Please print your completed form if you would like a copy for your records.
STEP 1
Please submit the Course Application form together with the:
Personal and Medical Information form and Participant Declaration form

STEP 3
Prior to your course review the essential course information which includes:
- location map
- items to bring to the course
- accommodation information
- travel details
